Background: Wound healing is a complex process and a worldwide challenge. Some studies have suggested light cure as a modality to accelerate wound repair. It could induce fibroblast proliferation, increase collagen synthesis and activate cellular processes involved in expression of procollagen type I and III mRNA. This study was designed to assess macroscopic effect of halogen dental curing blue light on a full thickness open wound healing in NMRI mices. Methods: 40 male NMRI rats were divided into the control and treatment groups. Full thickness wound with 6 mm diameter was induced on the lower back of all mice under general anesthesia and sterile conditions. All mice of the treatment group were exposed daily to a halogen dental curing blue light with a wavelength of 420-500 nm, for 7 days. The diameter of the wound was measured in both treatment and control groups every 2 days up to day 14.Findings: There was a significant difference in wound diameter between control and treatment group at all measurements after day 3 (P£ 0.05).Conclusion: Previous studies have shown the efficacy of low level laser in wound healing, pain relief, increasing local blood supply, decreasing inflammation and bactericidal effect. The results of this study suggest improvement of full thickness wound healing by daily irradiation of halogen dental curing blue light with a wavelength 420-500 nm for 7 days.

Background: Streptokinase is the only available thrombolytic agent in some areas. Streptococcal infection induces streptococcal antibodies and these antibodies increase failure rate in response to Streptokinase. This study was designed to investigate the effect of neutralizing antibodies on therapeutic effect of streptokinase to treat lumen cuffed catheter occlusion. Methods: In this study, 70 hemodialysis patients with thrombotic occlusion of double lumen cuffed catheter were studied. Demographic data, underlying disease, dialysis center, catheter size and location, duration of occlusion, history of catheter infection were collected. Blood sample was taken to determine neutralizing antibody before streptokinase injection. Streptokinase (50000 units per ml) was injected to the catheter lumen and was drawn after 24 hours. In order to occlusion documentation, ca radiography of chest and a radiography with contrast was taken before and after streptokinase prescription.Findings: The mean duration of obstruction was 4.4 ± 4.5 days. Neutralizing antibody was positive in 19 (37%) patients. Occlusion was resolved in 63 (90%) patients after streptokinase injection. There was no difference between patients with and without neutralizing antibody in response to Streptokinase (P=0.324). It was no relation between antibody positivity, infection history, duration of obstruction and response to Streptokinase. Conclusion: Streptokinase is an effective thrombolytic drug for management of double lumen cuffed catheter obstruction. Presence of neutralizing antibody has no effect on response to Streptokinase.

Background: Postoperative nausea and vomiting is a common complication that cause patient’s illness, increase of hospitalization duration and costs, and in severe cases lead to dehydration, electrolyte imbalance, aspiration and pneumonia and even opening of surgical site sutures. Giving attention to positive effect of Ondansetron in decreasing this complication we decided to evaluate its effect as prophylaxis and compare it with Dexamethasone as a gold standard and routine agent. Methods: This project is an interventional study that has done in year 2010 in Alzahra hospital of Isfahan, Iran. In this study 64 women older than 40 years whose candidate for an elective cholecystectomy surgery were studied in 2 groups. Patients were divided randomly to 2 groups: case and control.Control group received Dexamethasone and case group received Ondansetron before surgery.4, 8, 12 and 24 hours after surgery the incidence of nausea and vomiting in patients were compared between 2 groups.Findings: The frequency of nausea and vomiting in 4, 8 and 12 hours after surgery was significantly lower in Ondansetron group but in 24 hours after surgery there was no difference between two groups.Repeated measure ANOVA test also demonstrated that the trend of improvement of nausea and vomiting in 24 hours after surgery was significantly better in Ondansetron group. Conclusion: According to results of this study, Ondansetron has optimal effect in control of nausea and vomiting after elective cholecystectomy surgery and if there is no contraindication, we can prescribe it as prophylaxis.

Abstract:

Background: Bioaerosol is defined as airborne particles such as living organisms include bacteria, viruses, fungi and their related metabolites, such as endotoxin. The exposure to bioaerosols in the hospital may causes infection especially in immunocompromised patients. The purpose of this study was to investigate the Bioaerosol concentrations in air of various parts of three hospitals of Isfahan University of medical sciences, Iran. Methods: An Anderson biosampler was used for the bacterial and fungal collection. The effect of factors such as humidity, temperature and outdoor Bioaerosol concentrations were also evaluated.Findings: The lowest mean of fungal and bacterial concentration was observed in hospital operating rooms and the highest concentration was detected in infectious diseases ward of hospital 1 and 2 and surgery ward of hospital 3. Significantly, bacterial levels were found to be higher in hospital wards than outdoor, except hospital operating rooms. Conclusion: Our results show that bioaerosol concentrations in hospitals were relatively high and in particular, the higher levels of bacteria than outdoor might be related to the presence of patients, their activity, unsuitable ventilation and disinfection. Therefore, environmental control measures are needed to assure hospital environmental quality especially in wards with immunocompromised patients.

Abstract:

Background: Epidemiological studies indicate that elevated serum levels of homocysteine and C-reactive protein (CRP) is independently associated with risk of cardiovascular disease. The aim of this study was to determine the effects of 12 weeks aerobic training on levels of serum homocysteine and CRP levels in obese women. Methods: 20 healthy obese female students with similar age and weight were randomly devided in intervention and control groups. The intervention group was trained in an aerobic exercise program for 12 weeks (3 sessions weekly). This program was a treadmill exercise with 50-65 percent maximum heart rate intensity for 30-45 minutes in each session. Weight, body fat percentage, body mass index and waist to hip ratio were measured in the beginning and the end of the study for all of the subjects by using the analysis of body composition. In addition homocysteine and CRP levels in the fasting state were evaluated with ELISA (enzyme–linked immunosorbent assay) method before and after the exercise program. Data were analyzed by the Paired t-test and covariance analysis test.Findings: The mean age in both groups were 25 ± 5 years. The results showed a significant decrease in weight, fat percentage, body mass index, waist to hip ratio parameters and homocysteine and CRP levels in exercise group after 12 week exercise period. At the end of the study we found statistically significant differences in weight, fat percentage, body mass index, waist hip ratio parameters and homocysteine and CRP levels between 2 groups. Conclusion: According to our result, a 12 week regular aerobic exercise program, with moderateintensity play an important role in weight control in obese individual and decrease their cardiovascular disease risk factors.
