Abstract: 

داروهایی که مورد سو استفاده قرار می گیرند از جهت ساختمان مولکولی و مکانیسم عمل سلولی شباهت بسیار کمی با هم دارند. آنچه که مورفین؛ کوکائین و باربیتوراتها را در یک دسته قرار می دهد؛ احساس سرخوشی است که پس از مصرف این ترکیبات فرد را متمایل به مصرف مجددشان می سازد.این داروها دارای اثرات تولرانس؛ وابستگی جسمی؛ وابستگی روانی و شرطی شدن هستند که بازتاب آن در اعمال فرد مشاهده می شود.مهمترین مسیر عصبی که در ایجاد وابستگی دارویی دخالت دارد نورونهای دوپامینرژیک مغز می باشد. این راه عصبی از حفره مغز میانی شروع شده و به منطقه لیمبیک ختم می شود. کلیه ترکیباتی که ایجاد وابستگی دارویی می نمایند آزادسازی دوپامین را در این مسیر افزایش می دهند.در ایجاد وابستگی دارویی عوامل روانی - اجتماعی و بعضی فاکتورهای ژنتیکی به اندازه مکانیسمهای نورو فارماکالوژیکی موثر هستند. بنابراین دارو درمانی تنها یک جز از روش های درمانی است که می توان با استفاده از آن به درمان اعتیاد نایل شد.

Abstract: 

بسیاری از پزشکان در ویزیتهای روزمره با شکایاتی از قبیل درد معده، احساس سنگینی و عدم هضم غذا، افزایش گازهای شکمی و تهوع و استفراغ مواجه هستند. این شکایات در طیف گسترده ای از مراجعه کنندگان اعم از زن و مرد در سنین مختلف مشاهده می شود. تصمیم گیری برای نحوه برخورد با این شکایات، مشکل و گاهی حتی برای متخصصین گیج کننده است. سوالات مهمی در این زمینه وجود دارند: آیا باید بدوا اقدام به درمان علامتی کرد یا می بایست بررسیهای بیشتری انجام داد؟ در کدام گروه نیاز به بررسی های بیشتر وجود دارد و این بررسی ها باید شامل کدام آزمایشات و روش های تصویر برداری باشد؟ در صورت اقدام به درمان علامتی کدام داروها با کدام مکانیسمها مناسب هستند؟    این نوشتار با مروری بر آخرین مقالات در زمینه سو هاضمه سعی در رفع این ابهامات و ارائه آخرین شیوه های برخورد با این شکایت شایع دارد. در ابتدا با تعریف دیس پیسی بطور عام آشنا شده و مهمترین علل آن مختصرا برشمرده می شوند، سپس بر موضوع اصلی یعنی سو هاضمه بدون زخم متمرکز شده و به اتیولوژیهای آن اشاره می شود.  در مرحله بعد شیوه برخورد با بیمار دیس پیسی آورده شده و همچنین آزمون های مختلف مرتبط و مورد نیاز از جنبه های گوناگون با هم مقایسه شده اند. در پایان جدیدترین درمانهای موجود و موثر مورد بحث قرار گرفته اند. در طول این نوشتار با توجه به نقش موثر هلیکوباکتر پیلوری در بیماریهای دستگاه گوارش فوقانی نگاه ویژه ای به آن شده است. در انتهای این مبحث برای برآورد میزان فراگیری خوانندگان تعدادی پرسش چند گزینه ای مطرح شده است.

Abstract: 

A new antiproliferative activity from the conditional medium of two androgen-independent prostatic cancer cell lines, PC3 and DU-145, was identified. This antiproliferative activity selectively inhibited cell proliferation of an androgen-dependent prostate cancer cell line LNCaP in a dose dependent manner. No antiproliferative activity was observed against mouse fibro blast 3T3, normal human lymphocytes, human leukemic cells including promyelocytes HL-6 and T-cells HUT-78, or human adenocarcinoma cell lines, including prostatic cells JCA-1 ovary NIH: OVCAR-3, cervix C-33A and breast MDA-MB-231. Cell cycle analysis revealed that the antiproliferative activity did not induce apoptosis in LNCaP cells, but it prevented some of G1 LNCaP cells from entering into the S-Phase of the cell cycle. The antiproliferative activity was sensitive to protease digestion and its approximate molecular weight was between 50-100 kDa by Amicon filters. The antiproliferative activity was not affected by neutralizing antibodies against TGF- b1,2,3 TNF-a, PDGF, EGF, IL-1 IL-2. IL-3, IL-4, or IL-6.

Abstract: 

This syndrome was first described almost 40 years ago by Rubinstein and Taybi in seven children with broad thumbs and toes, facial abnormalities (beaked nose, heavy eyebrows, long eyelashes, hypoplastic maxilla, and a narrow, high arched palate), joint laxity, hirsutism, microcephaly, and mental retardation. By 1996, more than 600 patients with this syndrome had been reported. The majority of cases are sporadic. In 25% of the cases, submicroscopic deletions in, and chromosomal study is normal. In 12 16P 13.3 is identified. Mutations have recently been identified in non- deleted RTS patients. One case is a 6- year- old girl that was presented to us due to a speech delay. In physical examination, there are microcephaly, hypoplastic maxilla, high arched palate, talon cusps in teeth, beaked nose, a thin upper lip, heavy eyebrows, long eyelashes, joint (especially elbow) laxity, broad and angulated thumbs with flat nails, hirsutism, a stiff gait, and mental, motor, social and language retardation. Laboratory studies, including a brain C.T. scan, EEG, metabolic and chromosomal studies, and routine tests, were normal.

Abstract: 

Speech due to having specific setting in making communication between man and environment, has a special importance. Since disorder in speech causes a lot of problems, so one of the common aspects of speech disturbances is articulation disorder. In this study, relation between fine motor skills and articulation disorder has been investigated. This study has been carried out upon tow 7-10 year - old children groups; 46 children in case control and 36 with having disorder articulation. In this investigation, in order to survey the rate of children's fine motor skills diodochockentic test was used to evaluate the ability of fine motor skills in spech organs and Lincoln Ozerestky motor skills test was made in use to evaluate the ability of the children's fine motor skills individually; such as skills of fingers coordination of eyes, hands, arms, feet and body. In this study, it was observed that between the average of the obtained scores in the ability of performing the fine motor skills by using diodochocknetic test and Lincoln Ozerestky movement test in normal and abnormal children there has been significant difference (P<0.05). This research indicated that ability of performing the fine motor skills in normal children is more than ones with the articulation disorders. And in both groups, it was also determined that this skill is developed with the articulation disorders. And in both groups, it was also determined that this skill is developed with increasing the age.

Abstract: 

The importance of early diagnosis and treatment of these patients due to the significant complications of the lack of or delay in treatment in congenital hypothyroidism was studied in order to determine the frequency of congenital hypothyroidism among neonates with prolonged indirect hyperbilirubinemia. This descriptive study was performed on 500 neonates with prolonged indirect hyperbilirubinemia who had been referred to non-private and private clinics in Kashan from 1995 to 1997. Five (1 %) of 500 neonates suffered from hypothyroidism. These cases consisted of three females and two males. All the affected newborns were aged 15 days or older. Four of them were born at term and the other was pre-term. Three other cases showed a transient hypothyroid status. This study revealed a high frequency of congenital hypothyroidism in Kashan. Therefore, considering the large economic and social burden imposed by these developmentally retarded individuals on society, special attention is warranted in this regard.

Abstract: 

Because of an increase in the elderly population, attention towards the elderly has become S priority. To investigate the effects of social relationships and self-confidence on the mental health of the elderly, 100 males aged 65 or over were randomly selected cluster sampling. All of the males were from the central area of the city of Kerman. The group was evaluated on the measure of their social relationships and self-confidence. Five men were excluded from the study because of non-cooperation, and 7 were excluded for illiteracy. The GHQ 12 questionnaire was administered to the remaining participants. An analysis of the data using two-way ANOVA indicated that the obtained F value is not significant for any of the main factors of social relationships, but the F value for the interaction of the two variables of the study was significant (P<0.01). Thus, the simultaneous effect of social relationships and self -confidence on the improvement of mental health in the elderly is confirmed.

Abstract: 

Our aim was to determine the nature and extent of cardiac involvement in a group with thalassemia major who use DFO and to compare their cardiac status with that of nonthalassemic children. We reviewed 67 Patients with thalassemia major and the same number of nonthalassemic children and analysed factors affecting cardiac involvement. Factors studied included: age, serum ferritin, age of treatment onset, compliance to DFO treatment, electrocardiograms, chest X ray, echocardiograms. Ejection fraction (EF) and shortening fraction (SF) of thalassemic group were lower than that of nonthalassemic ones and their difference was significant (P<0.001). In thalassemic patients there was inverse relationship between serum ferritin and cardiac EF, SF. Patients with optimal compliance had lower serum ferritin (P<0.001).16 Palients showed some kind of cardiac involvement. They were older (P<0.001) with lower SF (P<0.05), and late onset of DFO (P<0.001). We conclude that patients with thalassemia major, despite iron chelation therapy with DFO, had weaker cardiac performance in compare with nonthalassemic ones. In these patients cardiac complications occurred in older age group with late onset of DFO. Echocardiographic measurement of SF has prognostic value.

Abstract: 

Noise can have many physiologic and psychologic effects. It causes derangement in peoples works, Activities, communication and sleep. In spite of advances in technology, researches show that the level of noise in hospitals is above internationally recommended levels and has many unwanted effects on staff and patients. The main sources of Sound pollution in hospitals are equipments noise and talks among personnel. This article discusses the main sources and effects of noise in three surrounding: the operating room, the recovery room, and the intensive care unit. Some guides in order to reduce sound pollution are also presented.

Abstract: 

Morning report is a traditional ritual within academic health care services. To compare the initial diagnosis of cases presented at morning report with the final diagnosis reached at discharge from the hospital, and the factors influencing this process, a survey on morning reports in one of the teaching hospital was conducted. This cross - sectional study was performed in the winter of 1997 among the 104 patients presented at the morning reports 84% had a definite diagnosis at the time of discharge. In 67.8% of the initial diagnosis at the morning report and the final diagnosis at the time of discharge were the same. The quality of presentation and the problems in the presentation of the patients were the only significant factors in this study. The experts believe that many parts of traditional educational medical interventions, such as morning report, need to be "updated" to reflect the new realities of medical practice.

Abstract: 

This Study was performed in descriptive method to determine the factors of disinclination to vasectomy in the health and treatment centers of Tabriz in 1996. The population consisted of 182 married men who were referred to tose centers for different health care reasons. The results showed that 70.3% refused vasectomy because of reluctance to become barren because of the risk of the inability to reverse the procedure. Of those surveyed, 17.6% refused because of pride and social problems, 14.8% refused because of the fear of reduction in sexual tendencies, 15.9% believed that it may bring some sort of advese effects, and 11 % did not have adequate information about it. From all of the confruet individuals, only 34,6% were ready for vasectomy. Among married men, 49.4% prefer tubectomy as a more suitable method of birth control. Over 61 % of individuals in the population accept vasectomy after their third child. The pride of men and the social problems related to the topic of vasectomy need more study.
